HPACK WGLC feedback: appendices

We currently have:

> A.   Change Log (to be removed by RFC Editor before publication)

This should be last (so that nothing else moves when it is removed)

> B.   Static Table Definition
> C.   Huffman Code

If these continue to be appendices, we'll have at least one IESG comment 
about having normative stuff in appendices. (I personally think it's 
fine, but we can avoid that discussion by moving them).

> D.   Examples
