iForms
To be eligible for Forms-at-Home, you must make an appointment with a participating provider. You will need to provide a valid email address and cell phone number to receive your forms.
Yes, sharing a phone number won’t affect your eligibility. Confirmation codes will go to whichever phone number is provided.
No, each individual must have a unique username to participate in Forms-at-Home.
Only the fields highlighted in yellow on a given form are mandatory. Once a specific form is completed, you will see a "Next" button at the bottom. Clicking this will take you to the next form.
Once all your forms are complete, you will see a "Submit" button at the bottom; once you click this, you're done.
Yes, you will get two reminder emails to complete your forms: five days before your appointment and the day before.
It may take a minute or two to receive your email. Be sure to check your spam folder if you did not receive your email; it will be from [email protected]. If you've confirmed you have not received this, please call (888) 321-3627.
Yes, if you are unable to complete your forms at home, you can complete them in the office on your own mobile device or an office tablet. You can also fill out your forms on paper, if needed.
No, these are standard questions that Experian asks to confirm your identity. They have no impact on your credit score.

Yes. If you cannot complete all the forms at home or have questions, you can finish any incomplete forms in the office.
Currently, our digital forms are available only in English.
