Am I eligible to fill out my forms at home?

To be eligible for Forms-at-Home, you must make an appointment with a participating provider. You will need to provide a valid email address and cell phone number to receive your forms.

Can my partner and I make appointments using the same cell phone number?

Yes, sharing a phone number won’t affect your eligibility. Confirmation codes will go to whichever phone number is provided.

Can a shared username be used to complete Forms-at-Home?

No, each individual must have a unique username to participate in Forms-at-Home.

Do I have to fill out all the fields on a given form?

Only the fields highlighted in yellow on a given form are mandatory. Once a specific form is completed, you will see a "Next" button at the bottom. Clicking this will take you to the next form. 

Once all your forms are complete, you will see a "Submit" button at the bottom; once you click this, you're done.

I can't fill out my forms right away. Will I get a reminder in case I forget?

Yes, you will get two reminder emails to complete your forms: five days before your appointment and the day before.

I didn't receive an email to complete my forms.

It may take a minute or two to receive your email. Be sure to check your spam folder if you did not receive your email; it will be from [email protected]. If you've confirmed you have not received this, please call (888) 321-3627.

I'd rather fill out my forms in the office. Is that OK?

Yes, if you are unable to complete your forms at home, you can complete them in the office on your own mobile device or an office tablet. You can also fill out your forms on paper, if needed.

If I'm asked to verify my account via Experian, will those questions affect my credit score?

No, these are standard questions that Experian asks to confirm your identity. They have no impact on your credit score.

Is it OK to fill out some of the forms at home and some in the office?

Yes. If you cannot complete all the forms at home or have questions, you can finish any incomplete forms in the office.

What languages are the forms available in?

Currently, our digital forms are available only in English.

Why is one of my appointments eligible for Forms-at-Home, but not another?

iForms is currently available only with specific doctors and practices. If you call (888) 321-3627, one of our agents can confirm whether your appointment is eligible.
